Membership Policies:

1. Clients may choose a membership with commitment periods of the choice above. One year recurring $59 and up contract will receive an additional 1 hour free gift certificate for the service of their choice from the above plan, free gift certificate may be shared or gifted.
2. Members’ Dues are paid thru automatic withdrawal (recurring billing) with a valid current credit card or checking account. (Note: A $19 fee will be applied for each credit card denial). It is up to the member to maintain account balances and current credit card information.
3. All memberships will continue as recurring from month to month after commitment ending date at the selected monthly plan except Three month non-recurring members. Members may cancel their membership after the first recurring bill date following the commitment ending date with 30 day written notice. Upon expiration or cancellation, unused complimentary services are lost and have no cash value.
4. Twelve month memberships must be in effect for at least six months, after which, a fee of $75 will be charged for early cancellation. Three months and six months memberships may not be cancelled. Cancellation request must be in writing. Return all requests to Salon O Organic Day Spa.
5. Membership is Valid only for contracted member (and Membership Plus members) listed at time of contract signing and cannot be transferred, or used by anyone other than member.
6. Monthly membership fees must be current and paid in full to receive complimentary services.
7. 8 hours notice required for cancellations or member will forfeit free massage.
8. Management has the right to terminate this agreement at any time.

MEMBERSHIP PLUS

The plus feature is added to the original Membership Agreement for a period of not less than six (6) months. The Plus Feature allows primary member to share membership privileges with one other member who is living in the same household.

• Member privileges can transfer between the primary member and add on member(s) only.
• Only the primary or add on member(s) can share or transfer service(s).

MEMBERSHIP BENEFITS:

a) Each month membership fee entitles the member to one of the package(s) selected.
b) Prepaid members may use services anytime during membership.
c) Unused service will be “Rolled-Over” to the next month.
d) Monthly service is a minimum of 60 minute session. It can be extended to 90 minutes for an additional fee.
e) Members may purchase unlimited massages for themselves but their guest and gift certificates are limited to two (2) for anyone person within the last six month period.
f.)Membership plans are offered in increments of 6 months, or 12 months.

Membership plan rates may increase at any time, but your rate is locked in for the life of your membership. At client request, plans may be deferred twice during the lifetime of the membership. Deferment may be up to 3 months. Additional discounts and promotions may not be combined with your reduced member rate.

Membership plans are provided at a significantly reduced rate and are therefore are non-transferable and may not be shared.

Client no-shows or late cancellations (less than 8 hours prior to appointment) will be forgiven once. After the first incident all no-shows or late cancellations will be billed at the member rate.

REFUND POLICY: All Sales are final. If buyer does not honor the contract, all promotional discounts must be paid in full.
